NEW CLIENTS

   All new clients must meet with our Provider, in-person or virtually, for a health & beauty assessment before receiving treatment. There is a $50 consult fee that will be applied towards your first treatment.  

PHOTOGRAPHY 

   Before and after photos are required for medical documentation. Clients may opt-IN (consent) or opt-OUT of photo and video image sharing on our social media platforms. 

PAYMENT

   Onyx requires a credit or debit card to be saved to your account to book all appointments. Credit and debit card information is kept securely on file.   Payment in full is expected at the time of service. 

CANCELLATION / NO  SHOW    

   In the event that you need to cancel or reschedule your appointment, you must notify us within 48 hours before. If you fail to do so or no-show your appointment you will be charged a $50 fee. 

REFUND

   Services are non-refundable, non-transferable to another person/product/service, and cannot be redeemed for cash or credit. 

ALL NEW PATIENTS REQUIRE A FULL CONSULTATION

We’re thrilled to announce that we have revamped our consultation process to provide patients with a more personalized experience. Our new holistic approach will focus on your individual needs, with a greater emphasis on your aesthetic goals, education, and long-term desired outcomes.

We will work with you to develop an annual treatment plan that is tailored to your specific needs. This comprehensive approach ensures that we can provide you with the best possible care as a whole person, rather than just treating your symptoms in isolation.

CONSULTATION FEE – $250

Your consultation fee can be applied towards your treatment within the first 30 days. When time allows, some treatments can be done immediately after the consult. If you choose NOT to have any treatment with us, the consultation fee will be applied towards the provider’s time.

LATE ARRIVAL/CANCELLATION & NO-SHOW FEES – $100

Late arrival, cancellation and no-show fees can NOT be applied towards future treatment or products. If we are unable to process the charge to the card on file, you must provide another form of payment to settle this outstanding fee before future appointments can be made.

Our accepted payment methods include cash, debit, or credit card. 
 

We do not accept Care Credit payments at this time. We offer a VIP Membership program which allows you to save money on your aesthetic treatments and plan for future treatments using a “beauty bank” wallet with our office.  

Additionally, we offer patient financing through our Cherry payment plans. With Cherry, you can choose from our most popular treatments and pay for them at a later date with individualized low monthly payments.
