By using the Onyx Medical Aesthetics website, booking an appointment, or purchasing a product or service, you agree to the following terms.

Medical Disclaimer

Website content is provided for general educational purposes and is not a substitute for personalized medical advice, diagnosis, or treatment. Submitting a form or requesting an appointment does not establish a provider-patient relationship. Treatment eligibility and recommendations are determined after an appropriate consultation and medical evaluation. Individual results vary and are not guaranteed.

New Patient Appointments

New patient appointments require a $250 nonrefundable and nontransferable deposit at the time of scheduling. The deposit may be applied toward eligible services or products purchased within 30 days of the appointment.

Cancellations

Onyx requires at least 48 hours’ notice to cancel or reschedule. Late cancellations and missed appointments may be charged a $100 fee. Select treatments may require an additional deposit.

Purchases and Promotions

Prices, services, products, and availability may change without notice. Gift cards, wallet credits, memberships, packages, and promotional offers are subject to the terms provided at purchase. Medical eligibility is required for all treatments.

Product, package, and membership purchases are subject to Onyx’s applicable return, refund, and cancellation policies.

Communications

By providing your contact information, you agree that Onyx may contact you regarding appointments, purchases, and requested services. Marketing messages may be opted out of at any time. Message and data rates may apply.

Do not use email, text messaging, social media, or website forms for urgent medical concerns.

Privacy

Information submitted through this website is handled according to our Privacy Policy, Consumer Health Data Privacy Policy, and Notice of Privacy Practices, when applicable.

All website text, photographs, graphics, videos, logos, and branding belong to or are licensed to Onyx Medical Aesthetics and may not be copied or used without written permission.

Onyx makes reasonable efforts to maintain accurate information but cannot guarantee that all website content will always be complete, current, or error-free.

Late Arrivals, Cancellations, and No-Shows

We kindly ask that you arrive on time for your appointment. If you arrive 15 minutes or more after your scheduled appointment time, we may need to reschedule your appointment, and a $100 fee may be charged.

Appointments canceled or rescheduled with less than 48 hours’ notice, as well as missed appointments, may also be subject to a $100 fee.

We understand that unexpected circumstances happen. Exceptions may be considered at the discretion of Onyx Medical Aesthetics.
